Concussion: Four Year Anniversary will feature professional wrestling matches involving different wrestlers from pre-existing scripted feuds, plots, and storylines that play out on AEW's television programs, Concussion. Wrestlers will portray heroes (faces) or villains (heels) as they follow a series of events that build tension and culminate in a wrestling match or series of matches.

The main match is scheduled to be Samoa Joe, Roman Reigns and Seth Rollins vs. Beat Down Clan (Montel Vontavious Porter, Kenny King and Low Ki) vs. Sons of Sandow (Kurt Angle, Mr. Anderson and Luke Harper) vs. Switchblade Conspiracy (Dean Ambrose, Sami Callihan and Jeff Hardy) in Hardcore Elimination Gauntlet, combining various feuds into one match.

At Redemption, AEW Intercontinental Champion Hiroshi Tanahashi fought Dolph Ziggler. It was announced that the two would face off again on the Fourth Year Anniversary of Concussion this time in a non-title 30-minute Iron Man match.

At Superclash IV Alexa Bliss won both the AEW Women's United States Championship and AEW Women's Championship in a Winner Take All Ladder match, it was later announced for the Four Year Anniversary Concussion that former Women's United States Champion Kelly Kelly will face Alexa Bliss for both the Women's Championship and Women's United States Championship.

Another match scheduled for the Anniversary Show is Conor McGregor and Brock Lesnar to face off against Kevin Owens and John Cena. Leading into this match McGregor and Cena have been in a feud. At Superclash IV, Cena defeated McGregor. As Lesnar and Owens have been in a feud as well, at Redemption Lesnar faced Heyman's new client, Cena. It would later be announced that McGregor would team with Lesnar and Cena would team with Owens.

At Redemption, Batista returned to the promotion, but was interrupted by The Miz, Batista would attack the Miz. It would soon be announced that Batista would face Miz at the Third Year Anniversary show.
