Saya Otonashi is a complex individual. She effectively has two sides to her personality that seem at times, contradictory and at odds with each other. However, when one finds out more about her past and background, the seeming division in her personality is understandable. Saya's 'human' side is very like the 16 year old girl she resembles. She is cheerful, optimistic and kind. She is able to heal all her injuries very quickly but must regularly receive blood transfusions or drink blood to remain healthy. She also can spend decades hibernating in a cocoon, only awaking for a few years (two to three, at least) before sleeping again for three decades and whenever her and Diva's blood mix, the blood crystallizes and disintegrates. She is also in possession of a rather large appetite, with a fondness for rare or even raw meat...
This brings us to her 'vampire' or chiropteran side, it is cold, calculating and has a warrior's edge that takes no foolishness (Her suppressed Chiropteran side is in evidence whenever her eyes glow red, in contrast to her sister Diva's glowing blue ones). When she or someone she cares about is in danger, or recently as she has learned to call on it at will, the 'vampire' will come forth, using her nihontō to dispatch enemies. Afterwards, she will usually revert back to her 'human' side, sometimes remembering what she has done, but also sometimes with partial amnesia.

Heals quickly, must drink blood or have blood transfusions to survive, above normal strength for a girl her size and speed. Night vision, above normal reflexes. She is very athletic and an expert in swordplay and martial arts. There are the fangs for the blood thing as well.

Saya by all appearances is a high school girl living a normal life on the island of Okinawa in Japan. She is the adopted daughter of a single father, George Miyagusku with two adopted brothers, Kai (18) and Riku (13). The oddest thing about her is that she seems to have no memory of her life earlier than one year ago when George took her into his family.
Saya’s happy life is shattered when she is attacked by a chiropteran, and learns that she is the only one who can defeat them. Her blood is poison to them and the only thing that can kill them. She denies what and who she really is at first, only to come to the realization too late to save her adopted father from a fate as a turned chiropteran. Having to kill her own father with her blood, she, and her servant Hagi who has appeared seemingly from nowhere, resolve to destroy the chiropteran and find the source of the madness. However the madness is closer to home than Saya knows about at the time..
Later on, she then starts having more flashbacks of her experiences fighting Chiropterans in the Vietnam War and in 1920's Russia. Her oldest memories are from her birthplace at Joel Goldschmidt's mansion called the "Zoo" in France during the 19th century.
She was "born" in 1833 from a cocoon like object taken from a mummy of a Chiropteran along with another cocoon containing her sister Diva. No one knows for how long she was in the cocoon before being released. She has no memories before it. Joel Goldschmidt was the man who found the cocoons and raised Saya as his daughter, deciding to use the other girl for his research and experiments. He kept the other girl locked in a tower, Saya didn't even know of her until later in life and when told, insisted on releasing her. She did and named her 'Diva' for her voice. However, Diva was not as Saya was and at a birthday party for Joel, she slaughtered him brutally. It is now known that the Chiropterans come from her blood. She has made it her mission in life to kill her sister and 'Red Shield' the legacy of Joel Goldschmidt, and also to have a family.
Saya is a skilled athlete and her weapon of choice is a nihontō (Japanese sword or katana, though one slightly changed to have features analogous to those of a kopesh) with bohi or grooves specially designed to spread her blood through its entire blade, making it a deadly weapon both for Diva and any Chiropteran made from Diva's blood.
Amshel breaks the first blade in the series, but she acquires a new sword from David with a red crystal at the base of its blade. This crystal is actually a piece of her adoptive father, George's blood which crystallized from when she killed him with her own blood.
However, lately, Saya has had very little to be cheerful or optimistic about as she learns more and more about herself and the enemies that stalk her. Her greatest fear is perhaps finding out that she is indeed the same or much worse than the very monsters she now hunts as hinted by the grisly yet splintered memories she has of her dark, shadowy past.
It is revealed that after she and Haji had left the "Zoo", she made him promise to kill her after she kills Diva to prevent the survival of Chiropterans. Halfway through the series, Saya gains a much darker appearance and personality, sporting a dark maroon trench coat and generally longer hair.
Past this point in the series, her willingness to co-operate with the Red Shield and with her own family drops, as she wishes not to endanger them any further, believing that her fight with Diva is hers to fight alone. Especially after the death of her brother Riku at the hands of her sister. Diva raped and murdered the thirteen year old in order to become pregnant and bear children, as she, and Saya as well, can only have children with each other's Chevalier. This also somehow marked a change in Saya's usually cheerful personality is also accompanied by an acceptance of her Chiropteran biology, with a willingness of necessity to feed off Hagi's blood.
Saya confronted her sister in the Metropolitan Opera House, and the duel ended when they pierced their swords through each other. When Diva began to crystallize, Saya survived as Diva's blood lost her potency due to her having her twins. At Kai's pleas and through Haji's admittance of love, Saya made a decision to live instead of die and to let Diva's twin girls live.
Shortly after her return to Okinawa, she went into her hibernation period, (most likely due to the stress of Diva's blood) and in an epilogue, Kai along with the twins watch over her as she rests in the Miyagusuku family grave. Haji, who was thought dead, also visited, leaving his hair ribbon tied around a rose.
