Extreme Rules will consist of professional wrestling matches that involved wrestlers from pre-existing scripted feuds or storylines that play out on IWE's primary television programs, Raw and SmackDown. Wrestlers will portray heroes or villains as they follow a series of events that build tension and culminate in a wrestling match or series of matches.
At Payback, Roman Reigns defeated KJ Styles to retain the IWE World Heavyweight Championship. However, Styles defeated Reigns by countout (which was restarted by Shane Matteson) and then it resulted via DQ, after Reigns low blowed Styles (which was restarted by Kaila Matteson). At Payback, The Vaudevillains vs. Enzo Amore and Colin Cassady ended in a no contest after Enzo Amore suffered an injury, which was later found to be a concussion, in a #1 contender match for the IWE Tag Team Championship. The following night on Raw, The Vaudevillains was award a title match against IWE Tag Team Champion The New Day at Extreme Rules.
At Payback, IWE Women's Champion Charlotte defeated Natalya in a controversy when Charles Robinson called for the bell when Natalya didn't tap out when Charlotte had her in the sharpshooter, similar to the Montreal Screwjob of Survivor Series 1997. The following night on Raw, it was announced that Charlotte would defend the title against Natalya in a Submission match with Ric Meyer banned from ringside.
