Extreme Rules is an upcoming professional wrestling pay-per-view (PPV) event produced by IWE, which will take place on April 29, 2012 at the Allstate Arena in Rosemont, Illinois. It is the fourth Extreme Rules event. There are eight official matches scheduled for the event's card.

Wrestlers portray villains or heroes as they follow a series of events that build tension and culminate in a wrestling match or series of matches.

On the April 2 episode of Raw, Dustin Simpson spoke about his loss to The Rock the previous night at IWE's WrestleMania XXVIII PPV event. After concluding his statement, he asked for the Rock to come out and talk. Instead, Rob Lesnar made his IWE return after being away since 2004. Lesnar deceived Simpson by teasing a handshake in order to execute his F-5 maneuver, dropping Simpson onto the mat. The next week, villainous authority figure John Laurinaitis revealed that he signed Lesnar back to bring "legitimacy" and for Lesnar to become the "new face of the IWE;" Laurinaitis also announced that Lesnar would face Simpson at Extreme Rules.
