Thinking about how you're gonna spend the rest of your life with the world being muted to you and only you, you act out in rage and selfishness, tearing through the door to the doctor's office with your punkish claws.
The doctor is startled - naturally - and acts like a stereotypical doctor being attacked in a horror movie; he stands up, waiting for the drooling teenage punk to attack him.
