Real Name - You can give a pseudonyme here, but remember that you will probably be called this.
Real Age - Self-explanatory.
eM@il Address - Only give this if you want me to send emails to another email address than the one you wrote the mail with.
Instant Messengers - give which ones (Gmail, AIM, YIM, ICQ, IRC etc.) and which addresses/nicks and for IRC which servers and channels to find you on.
Desired Ship - eg. USS Dagger (MX-5298)

Now you have to wait. I will log you on to the ship's site and you may enter your biography in the control panel. If I am happy with this, you will run through CO academy. If you pass this, you can take command in the rank of Lieutenant Commander or Marine Captain; this is if you are not in RIF (Romulan Integrated Fleet) or the Cardassian Associated Troops.
