AEW III: Reigns vs. Ambrose will feature professional wrestling matches that involve wrestlers from pre-existing scripted feuds or storylines that play out on AEW's television programs, Concussion. Wrestlers will portray heroes (faces) or villains (heels) as they follow a series of events that build tension and culminate in a wrestling match or series of matches.

At Superclash IV, Dean Ambrose had cost Roman Reigns his AEW World Heavyweight Championship. It was later announced by Paul Heyman that the January pay-per-view would be titled; AEW III, as well as announced that Reigns would face Ambrose at the event.

At Ultimate Ascension, Knux won the Triple Tier Cage match to win the AEW World Heavyweight Championship. After the match, John Cena cashed in Goldust's Money in the Bank contract and defeated Knux to win the World Heavyweight Championship. On the January 12 episode of Concussion, it was announced that Cena would make his first title defense against Kevin Owens at the event, however former AEW Champion, Colt Cabana returned to the promotion being announced as the new AEW DOA, firing Paul Heyman, before announcing that former champion Knux would receive his rematch against Cena instead at AEW 3.

At Ultimate Ascension, Taryn Terrell defeated Paige, Bayley, Dana Brooke, Charlotte and Eve Gracie to win the first Women's Money in the Bank ladder match. Later in the night after Candice Michelle had defeated former holder of both Women's Championships Alexa Bliss and Awesome Kong in a Triple Threat Ladder match to win both the AEW Women's Championship and the AEW Women's United States Championship, Tarrell cashed in her Money in the Bank contract to win the AEW Women's Championship. However, Women's General Manager AJ Lee announced that Tarrell would make her first title defense at AEW III against Charlotte.

At Devil's Night, Johnny Gargano defeated Knux to win the AEW Television Championship. On the January 12 episode of Concussion, Enzo Amore defeated Tyler Breeze to become the number one contender at Johnny Gargano's AEW Television Championship, the match was later revealed to take place at AEW III.

At Ultimate Ascension, Stardust attacked Fin Balor during a in-ring segment. On the January 12 episode of Concussion, it was announced that Fin Balor will faces Stardust at the Prelims of the event.
