Extreme Rules will feature professional wrestling matches involving different wrestlers from pre-existing scripted feuds, plots, and storylines that play out on IWE's television programs. Wrestlers portray heroes or villains as they follow a series of events that build tension, and culminate in a wrestling match or series of matches.

Unlike previous events, the main event will only be one Money in the Bank ladder match due to the unified championship. Normally, the winner of this match receives a briefcase with a contract that entitles him to a IWE World Heavyweight Championship match at any program at the date, place, and time of his choosing within the next year; however, on the June 9 edition of Raw, Kaila Matteson announced that the match would instead be for the vacant IWE World Heavyweight Championship itself, as previous champion Kody Brown would not be able to defend the championship at the PPV due to injury (and subsequent surgery) and was stripped of the title. On the June 2 edition of Raw, Alberto Del Rio qualified for the match by defeating Dolph Ziggler. On the June 9th edition of Raw, Triple K placed Kevin Orton into the match, and Sheamus and Cesaro defeated |Bad News Barrett and Rob Van Dam, respectively, to qualify for the match. At the following SmackDown taping on June 10, Bray Wyatt would qualify for the match after defeating Dean Ambrose (with interference from Seth Rollins).

During the Dustin Simpson–Bray Wyatt feud, IWE Tag Team Champions Jimmy and Jey Uso would insert themselves in the rivalry alonside Simpson to ensure his matches with The Wyatt Family were fair and to counteract Wyatt Family members Luke Harper and Erick Rowan. At Payback, the Usos would once again be in Simpson's corner to even the odds and in the process, Simpson defeated Bray Wyatt in a Last Man Standing match to culminate the feud. The next night on Raw, Rowan and Harper would shift their attention to the Usos. They then defeated the latter in a non-title tag team match. Due to this win, they earned a match the Tag Team Championship at the PPV.
