You tell the boy your name.
"I'm John!" he says, with a beaming smile, "Do you like my car?"
He holds up a red racecar for you to see. You amuse him by nodding.
You continue to chat with the boy for the last little bit of your life. No opportunites to save anyone arise during that time. As your last precious few seconds tick away you hear a nurse enter the waiting room. You look up as your time hits zero and are suddenly blinded by a bright white light that comes forth from somewhere behind her. You use your arms to shield your eyes, and thankfully the light passes quickly.
As you lower your arms, you realize you can't hear the boy speaking anymore. In fact, you aren't even in the hospital's waiting room anymore. Instead, you find you are in some kind of black abyss. There is nothing but darkness all around you and you can't even see what you are sitting on anymore.
A hooded black cloak suddenly rises from the ground in front of you. A featureless, white porcelain mask sits where its' face should be as it floats off the ground only a few feet away. You can see no body, or discernable form and where eyes should be you find only a deep darkness.
"There are worse ways to go than spending your final moments talking to the innocent," a voice says, coming from the phantom. It is the same voice that you heard an hour ago when you made a deal in that small room.
"But you haven't fulfilled your end of the bargain so I am here to collect what you owe me... your soul."
You are pulled from your seat, your legs moving against your will as you walk toward the phantom.
"I hope that you enjoy your time with me. I know I will enjoy my time with you..." the phantom says as it turns and floats away. A cruel, haunting laughter fills the abyss you're in as you walk.
You get a strong feeling that now that this phantom has you, it isn't going to let you go even if you pay off your debt...
Game Over: You belong to the Phantom now, and forever.
