504 forms and samples
From Afehandbook
Public School students are eligible for a 504 designation which allows a student with a disability to receive accommodations for learning in all school environments. Usually a meeting is convened with the student, parent(s), consultant teacher, another teacher representative, principal or other professional, and special education representative. Eligibility for a 504 is determined and accommodations are written up. This is a basic sample of a 504 and possible accommodations. Refer to the binder for a list of many possible accommodations for students.
