You heal her wound. Not all the way, but enough that she'll survive. And you make it so she can't get sick, because what would be the fun in that?
Hawkeye, after confirming that the other guy is dead, chases after Black Widow. He finds her in the shadows, ready to fight. She defeats him, but it is a rather close match, and she does not kill him.
They meet again, a week later. This time, he wins. But he does not kill her either. Instead, he takes the arrow out of her shoulder and nurses her back to health. Aww. It takes a while, but they become friends, and then lovers. Within two years they have taken over the empire. All hail Queen Natasha!
