Tuttlingen is a small city on the border of black forest (Schwarzwald), well known for its medical technlogoy and surgical instruments industry. In this sense, it is very similar to Sialkot, Pakistan.

Since 2005, on the first visit in the quarter, the patient must pay additional 10 euros as Praxisgebuehr. Each calendar year is divided into four quarters (Jan-Mar, Apr-Jun, Jul-Sep, Oct-Dec). Subsequent visits in the same quarter are not charged Praxisgebuehr. This is valid only for adults.
