Tamaki is the president and founder of the Ouran High School's first ever Host Club. The club involves affluent, good looking men of different stereotypes entertaining their classmates, mostly female. Tamaki started the group with best friend and vice president Ootori Kyouya who is in charge of most of the planning, though Tamaki is under the delusion that he organizes the club. While most events start as his bizarre fancies, Kyouya often turns them into successful and profitable experiences. Amongst all the members, Tamaki is one of the few who doesn't pretend his way through club sessions. He genuinely enjoys playing the playboy prince and means every compliment he bestowes on his customers. He is extremely popular within the school and academically is ranked high in his class, second only to Kyouya.

Tamaki is the one who comes up with the idea of recruiting Fujiyoka Haruhi into their ranks, though he is often conflicted by this decision when he realizes how cute she looks as a girl. He tries to introduce commoner concepts into the club now that she has joined. Haruhi is Tamaki's most coveted person and he often showers her with praise and physical contact. He is also extremely close to Kyouya, who appears to be one of the few people capable of handling Tamaki's theatrics. He does not conceal his irritation towards the twins Hitachiin Kaoru and Hitachiin Hikaru who tease him at every opportunity.

Although Tamaki seems as flawless as every other "rich bastard", his past isn't exactly perfect. Tamaki's father is a wealthy Japanese businessman (also the superintendent of Ouran) who fell in love with Tamaki's mother, a French aristocrat, and produced Tamaki out of wedlock. Though he divorced his original wife, he was forbidden by the Suoh grandmother from bringing his 'French harlot' and her child to Japan. However, after fourteen years the Suoh family became concerned about a family heir. Around this time Tamaki's mother who had been raising her son in France, lost the family fortune. Because she was extremely ill, the Suoh family proposed a deal; They would fund Tamaki's mother's medical treatment if she sent her only son to live in Japan as the Suoh heir, forbidden to ever have any contact with her again. Determined to take care of his mother, Tamaki agreed and left France. Whether to make the separation easier on her son or from her own guilt, Tamaki's mother disappeared soon thereafter. Tamaki lives in Japan as an 'adopted' child and is not permitted to live in the same mansion as his father or the rest of the Suoh family. Though his father is wildly affectionate, he is under the constant scorn of his grandmother, who often insults him to his face. In the manga, Tamaki tells a sympathetic Haruhi, "I am myself before I am a Suoh" and is able to carry on happily because of his personality.

Tamaki also holds the Host Club to a controversial set of family rules. He refers to himself as Haruhi's "father", Kyouya as "mother", and the Hitachiin twins Haruhi's villainous older brothers. Honey and Mori are dismissed as "just neighbors". The reasons for this are somewhat opaque. Tamaki states several times that clubs are like families, a thought possibly stemming from his own shaky home situation. Kaoru brings up the possibility that Tamaki calls them a family to prevent inter-club dating and preserve the innocent times they have together. In the anime, Tamaki shoots this theory down, admitting he doesn’t know why he calls Haruhi his daughter. What else could she be if he think she's so cute and adorable? Why else would he try to protect her from other boys? This is the magnum opus of Tamaki's denseness.

At the end of the anime series, Tamaki is engaged to a suspicious girl named Eclair Tonnere and plans to move back to France. Everyone (including Tamaki) is unhappy and shocked, excluding Tamaki's grandmother who was manipulating his desire to reunite with his mother. The Host Club forcefully intervenes and Tamaki happily remains in Japan with his friends. Despite their schemeing, he does not hold any bad feelings towards his Grandmother or Eclair- even if the rest of the Host Club is not so understanding.

Tamaki is an excellent pianist. He is completely obsessed with commoner things and Japanese culture. He owns a dog named Antionette.

In Purgatory

After locating Haruhi, Tamaki took up a residence at a fairly posh hotel until he was evicted due to a lack of funds. Kyouya arrived at the scene around this time and Tamaki agonized beside him over their sudden poverty. Kyouya told him to cut the shit, but in the sweetest, BFF way possible. The rest of the Host Club has since appeared as well, prompting Tamaki out of his destitute depression and into full Host Club planning gear.

Having collected enough of their stipend, Tamaki and Kyouya purchased an old dance studio at 413 Figaro Avenue (with a decent vie of the ocean!). They are currently attempting to renovate it into an acceptable venue for the Island Host Club's first annual Gala. No word on how this is possible, since the two have a decided lack on manual labor.

Tamaki spends much of his time staring out the window, wondering when Haruhi will come to visit.
