You leave the man's shack to see a empty street no buildings are standing on the street besides the shack/hospital you were just in and a bar with a neon sign that says "Ramona's" on what is clearly a pub down the street. The sun is high up in the sky and if you had to guess it's midday so to escape the sun and as per the man's instructions you enter the bar.
