Having failed to learn from experience, you pull the same lever a third time. This time, a large stone flies out of the hole. It smashes into your face and cracks your skull. You fall to the floor with your brains leaking out. As you lose consciousness, you could swear you hear Treguard's voice saying, "Ooh, nasty..."

Then you die.
